Please print out this form and submit together with registration fees or deposit:

Payment and Refund Policy: A $50 deposit or credit card hold reserves a seat for a student. Deposits are refundable one-week prior to the start of class. Sorry, but no refunds after that except in the event of cancellation of a class; credit only for comparable service within 30 days. If coupon or a discounted fee is used, the entire fee is due at that time. Only one discount applicable.

Company POs or vouchers are acceptable at net 30 days and a finance charge of 1.5% per month past due applies.

Please make checks or money orders payable to: Freehold Computer Center. There is a $30 fee on all returned checks.

Cancellations: Cancellations require 24 hours advance notice in order to be rescheduled or made-up within 30 days. One emergency same-day cancellation can be made per 18 hours of enrollment. Subsequent same-day cancellations are subject to charge without make-ups.

Company-only classes: Classes will be held regardless of the number of students able to attend once a class is scheduled.

The Center prides itself in being personable and being able to communicate technical know-how in an easy, understandable manner. So if there is anything in the above policies that is not clear, please, please, let's discuss it before there is any misunderstanding.
